By completing this registration form, I confirm that my child is enrolled at Saint John’s School for the 2025–2026 academic year, and that I'm sharing voluntarily this information with Envoys. I also acknowledge and agree that Envoys may receive and use this information, including confirmation of enrollment status, to facilitate the trip and related arrangements.

By submitting this registration, I authorize Envoys to share the information contained herein with Saint John’s School to verify my child’s enrollment status if necessary.

• Be as precise as possible when answering the medical information form. The more information we have, the better care we can offer for those traveling.
• Make sure the student's name matches their legal name as it appears on their passport.
